Low Back & Hip Pain? Is it Nerve, Muscle, or Joint? How to Tell.

Back pain often has low success rates when it comes to surgery compared to buttocks pain that has high success rates. So if surgery is not an option for you, I think the answer is yes try everything else.

Everything else can range from physical therapy to acupuncture to chiropractic care to stem cell therapy. I have had patients respond to every version of therapy, and I have had patients respond to none of these options.

I would absolutely recommend trying acupuncture to see how much relief that provides you.

The Diagnosis: Is It Your Hip

When you tell your doctor your hip hurts, the first thing she should do is confirm that your hip is actually the problem. Women might say they have hip pain, but what they may mean is that they have pain in the side of the upper thigh or upper buttock, or they may be experiencing lower back pain, says Stephanie E. Siegrist, MD, an orthopedic surgeon in Rochester, New York, and a spokeswoman for the American Academy of Orthopaedic Surgeons. Hip pain is often felt in the groin or on the outside of the hip directly over where the hip joint is located.

Treatment Options For Hip Pain

Treatment for hip pain depends on the diagnosis, but pain that’s caused by overuse or sports injuries is often treated with heat, rest, and over-the-counter anti-inflammatory medication. To prevent injuries, it is important to stretch before exercising and wear appropriate clothing, especially good shoes when running, Dr. Philippon says.

If certain activities or overuse are causing hip pain, stop those that aggravate the discomfort and talk to your doctor. Excess weight can put pressure on the hip joint, so losing the pounds can provide relief and help you avoid further problems. Some causes of hip pain, such as fractures or hernias, may need surgical repairs. If your hip pain persists, talk to your doctor about the possible causes and treatments.

Improve Movements To Eliminate Hip Pain

The back, hip, and lower extremity work as a comprehensive unit allowing for many of the repetitive tasks you complete at home, work, and during recreational activities. Injuries to one area of the musculature often indicates that additional damage has been incurred by adjacent muscles.

Many therapeutic exercises can help restore proper strength and endurance to the leg muscles. Isometric exercises are often the initial treatment exercises, followed by single plane rubber band exercises for the hip, knee, and ankle: flexion, extension, adduction, abduction, circumduction, inversion, and eversion. Dynamic exercises involving stability foam, rubber discs, exercise ball, and BOSU balls can be performed on the floor. The more unstable the surface, the more effort and stabilization is required of all the lower extremity muscles.

The Varieties Of Lower Back And Hip Pain

Hip vs. Low Back: Identifying the Source of Pain

Many of my clients experience lower back and hip pain simultaneously. But all too frequently they receive inadequate guidance from their physicians about possible causes and about how to relieve it in a lasting way.

When X-rays and MRIs come back negative, a physician without adequate training in the musculoskeletal system may not know what to prescribe other than physical therapy exercises.

The problem with this strategy is that PT can aggravate certain conditions. I’ll go into this in more detail with each of the conditions listed below.

The most frequent regions of pain I hear reports about include

  • The lower quadrant of the back
  • Across the top of the hip bone
  • Shooting into the sacroiliac joint
  • Deep in the gluteal muscles

  • Traveling down the leg

Often lower back and hip pain can present as a kind of moving target. Sometimes the pain is centered squarely on the outside of the hip joint near the hip bone.

Other times the pain can wrap around to the gluteal muscles. At times an individual might experience pain in more than one area.

The reason for this medley of possibilities is that muscular compensation is often at the heart of lower back and hip pain.

Further, muscular compensation is a continual process in which muscles are constantly adjusting, trying to figure out how to make do with the circumstances they’re presented with such as postural distortion.

Hip Arthritis Or Osteoarthritis

Wear and tear of your hip bone can cause inflammation in the hip socket which results in dull aches or, sometimes, sharp jabbing hip pain.

Researchers from the American Academy of Orthopedic Surgeons say that arthritis of the hip causes stiffness and pain. This can affect everyday activities like walking, bending over, or rising from a chair. Other associated symptoms of arthritis of the right hip can include:5

  • Pain that intensifies with physical activity
  • Hearing a grinding noise when you move the right leg or left leg
  • Pain that radiates from your right hip to your groin or thigh
  • Pain in the hip socket in damp weather
  • Tenderness and possible swelling in the hip

Some ways of reducing pain in your right hip caused by arthritis include losing excess weight, low-impact physical exercise, or physical therapy.

Starting position: Sit up straight in a chair and rest your left ankle on your right thigh above your knee. Place your hands on your thighs.

Movement: Keeping your spine neutral, slowly hinge forward from your hips until you feel a stretch in your left hip and buttock. Hold. Slowly return to the starting position. Repeat with your right ankle on your left knee. This is one rep.

Tips and techniques: Keep your spine neutral, not rounded, and your chest lifted as you lean forward. Keep your shoulders down and back, away from your ears, as you stretch. For a deeper stretch, gently press down with the hand on your bent leg.

What Are The Symptoms Of Hip Conditions

In general, people suffering from hip pain from hip pathology have symptoms that appear in the front of the hip and the groin area. Sometimes the pain will radiate into the front of the thigh but very rarely goes past the knee.

After sitting for a period of time, patients suffering from hip pathology find it difficult to walk when they first get up. The pain is worse initially, but after a couple of limping steps, the hip lubrication kicks in, and the pain subsides.

Sharp Pain In The Lower Back And Hip On One Side

A shooting and sharp pain felt on one side on your lower back and hip may be caused by muscle spasm, joint dysfunction, and/or nerve compression in the region. Common conditions that cause this type of pain are discussed below.

Peripheral Nerve Related Pain

The nervous system is a complex network of nerves and cells that carry messages between the brain and spinal cord and your body. It is through this system that we feel, move and control our bodily functions.

Nerve roots leave the spinal cord via the intervertebral foramina and join together from various levels of the spine to travel as cord-like structures, called nerves, to their destinations.

It is these nerves that travel outside the spinal cord that are referred to as peripheral nerves.

Some peripheral nerves travel only a short distance and others all the way from the lower back to the foot. Along their journey they run between and through muscles and fibrous tunnels.

While radicular pain arises from a problem as the nerve root exits the spine, nerve-related pain may develop due to a problem along the pathway of a peripheral nerve, outside the spine. Pain related to a nerve is called Neuralgia.

Neuralgia felt around the hip and pelvis may develop in many ways including excessive compression or stretch of the nerve. This may be caused by a sudden, acute mechanism, for example a fall or blow to the area resulting in compression, or the leg being caught and wrenched, resulting in stretch.

Alternatively, the onset may be subtle, with a gradual onset associated with sustained postures or repetitive movements that cause cumulative nerve irritation.
