What Is Glute Med Or Gluteus Medius

Gluteus medius or glute med is one of the three gluteal muscles in our buttocks. It starts at the surface of the ilium or the uppermost and largest part of our hip bone and runs in a slant or oblique position to support the strongest bone of our body which is the femur or thigh bones.

Gluteus medius is sandwiched between the two of the three gluteal lines or curved muscles that support the ilium and underneath the fascia lata. The inferior gluteal line is the less distinct muscle close to glute med but is more directed towards the spine.

What is Myofascial Pain in Buttock Muscles?

One of the most common causes of buttock pain is myofascial pain, which is characterized by pain starting from tight bands of muscle or knots in the gluteal muscles.

The gluteal muscles are strong muscles located at the back of the pelvis building up the buttock. The gluteals mainly comprise of three major muscles they are:

  • Gluteus minimus.
  • Gluteus medius.
  • Gluteus maximus.

Gluteus minimus, gluteus medius and gluteus maximus start from the pelvis and insert into the upper side of the thigh bone i.e. femur. Many smaller muscles are also present in the deeper side of the gluteal muscles such as the piriformis muscle.

The gluteal muscles help in straightening the hip while performing activities, stabilizing the pelvis and aiding in other movements of the hip like side elevation and hip rotation. Gluteal muscles are specifically active while squatting, lunging, running and jumping.

The piriformis muscles and gluteus medius are the areas that are very much prone to develop the trigger points associated with myofascial pain in buttock muscles.

Pelvic instability, lower back injuries and overuse of the gluteal muscles result in the formation of excessive tight bands of muscle called as knots or myofascial trigger points.     Weak Glutes And Low Back Pain

    The glutes can be weak, sometimes on one side and sometimes on both sides. When gluteal weakness is long-lasting, it causes various painful conditions and complicates daily activities.

    Weak glutes are often accompanied by chronic back pain while squatting and walking up the stairs are significantly impaired. How is this happening?

    I will try to describe it as simply as possible and step by step. I will start first with how glutes can become weak over time.

    Prolonged sitting and insufficient physical activity are the main reasons for weakening the gluteus. Prolonged sitting does not have an exact definition, but if a person sits for eight hours every day, that is too much sitting. By lack of physical activity, I mean a lack of activities that include squatting, stepping, running, and targeted leg strength exercises.

    What happens next? The body adapts to weak glutes so that the muscles at the front of the pelvis have to work harder. These muscles are hip flexors and they are antagonists to the gluteus muscles.

    The position of the pelvis changes and rotates forward. This phenomenon is known as anterior pelvic tilt. In this pelvic position, there is increased tension in the lower back. This pattern seems harmless, but over time it can create chronic problems in the lower back.

    Now I will talk more about how you can relax tense glutes and strengthen them so that your back becomes even stronger and healthier.

If a client has been released from therapy or their doctor and the pain persists, an effective corrective exercise strategy for correcting this type of dysfunction would be to first release the hip flexor through appropriate integrated flexibility techniques, then implement reeducation of the gluteus maximus through isolated strength training.

    Finally, recondition the lumbo-pelvic hip complex through integrated functional movements in the appropriate range of motion, plane of motion, and speed, specific to the needs of the client.
