What Causes Pain On Outside Of Knee

Pain on the outside of the knee can be caused by many things, but most of the time it’s not serious.

One cause can be a problem inside of the knee joint such as knee arthritis or a meniscus tear.

However, that type of pain is usually deep inside the joint and you can’t touch it with your fingers.

A pain right over the outside of the joint can be caused by a ligament sprain but that usually only occurs when:

  • there is an acute injury to the knee
  • the knee feels unstable, like it will give way

Proximal Tibiofibular Joint Dislocation

Proximal tibiofibular joint dislocation is one of the most unusual causes of lateral knee pain. It affects the joint between the top of the shin bone and the fibular, the small, thin bone that runs down the outer side of the shin, just below the knee joint on the outer side.

It takes a large force to dislocate the joint, e.g. a car accident, but it can also partially dislocate usually due to a fall when the foot is plantarflexed , which often also damages the tibiofibular ligament.

Symptoms usually include outer knee pain, instability especially during deep squats and sometimes an obvious deformity at the side of the knee. There may also be associated damage to the peroneal nerve leading to pins and needles or numbness around the outer knee.

What Is Iliotibial Band Syndrome

Iliotibial band syndrome is often called IT band syndrome. It’s a health problem that causes pain on the outside of the knee. It most commonly happens in athletes, especially distance runners, or those new to exercise.

The bones of your knee joint are your thighbone , your shinbone , and your kneecap . Your iliotibial band is a strong, thick band of tissue that runs down the outside of your thigh. It extends all the way from your hip bones to the top of your shinbone.

When you bend and extend your leg, this band moves over the outer lower edge of your thighbone. With repeated bending and extending of the knee, this movement of the iliotibial band may irritate nearby tissues, causing pain.

Another symptom of runners knee is a clicking, grinding, or rubbing sound when youre bending and straightening your knee.If your runners knee is due to IT band syndrome, the pain will most likely be present on the outer side of the knee. In that case, the pain will typically occur during running, and vanish as soon as you stop.

It is worth noting that non-athletes can develop runners knee as well. The pain and stiffness associated with runners knee can make it difficult to do everyday activities like kneeling and climbing stairs.2

Pain outside of knee is that pesky pain experienced around the lateral part of the knee that commonly affects runners of all levels of experience. The ITB is a long band of connective tissue or fascia that originates on the outside of the pelvis and extends down the outside of the leg to insert just past the knee joint. Its main function is to assist in stabilisation of the knee during weight bearing activities.


ITB pain most commonly presents on the outside of the knee around the bony prominence called the lateral femoral epicondyle. The pain may not initially present at the beginning of a run, however will usually intensify as the run progresses. Flexion of the knee through a range of motion past 30 degrees has been found to exacerbate the problem. Flexion of the knee to 30 degree causes the fascial band to rub across the bony prominence. Thickening and swelling around the outside of the knee are also common symptoms.
